Balance Costs And Benefits With 3500 Deductible PPO |
There are numerous health care plans in the market and balancing the premium costs with the return of benefits is something for which every person is looking. Being able to find that blend for only coverage people want and need is the goal of Blue Cross of California.
One of the popular choices for great benefits and lower premiums is the 3500 Deductible PPO. With this plan, once the annual $3,500 deductible per member has been reached, there are no charges for doctor’s office visits of visits for professional services, including X-ray, diagnostic lab and surgeon visits. Additionally, once the deductible for two members has been reached, the deductible for the entire family is considered satisfied.
Blue Cross negotiates with all service providers in its network for the lowest fees possible and the fees negotiated are accepted by the providers as payment in full for services rendered. The fees charged by non-network health care providers will be higher.
Hospital treatment, either inpatient or as an outpatient is also free to members once the deductible is met and there is no charge for emergency room services, after deductible is met. Services for maternity care are not covered under the 3500 Deductible PPO plan.
An additional $500 admission charge at participating hospitals will assessed. There is no additional charge for preferred participating facilities. The charge is for inpatient stays or outpatient surgery or infusions. It is not required for ambulatory surgical centers or for medical emergencies.
Prescription service for Blue Cross formulary drugs involves a $10 copay for generic drugs. For brand name drugs there is a $500 deductible after which the name brand drugs will require a $30 copay. Members choosing a name brand drug when a generic equivalent is available will be charged the generic copay plus the difference in the costs of the name brand drug and the generic drug, even if the physician noted on the prescription to dispense as written or no not substitute. Thirty percent of the negotiated fee will be charged for self-administered injectable drugs, except insulin.
